Police reports

Police Incident Reports (PIRs) and Vehicle Collision Reports (VCRs) can be obtained upon request.

Requests for police reports must be made using the form:

PD268 Application for a Police Incident report (PIR) or Application for a Vehicle Collision Report (VCR) (PDF icon 32KB

The application fee of $56.50 must be included with your application.
No fee waivers or concessions apply.

Forms must be submitted in person at your local police station.

Proof of identity is required and originals must be presented to a SAPOL employee when you submit the application.

For company vehicles, proof of registration and a declaration of the applicant's employment on company letterhead is required.

Proof of Identity

The applicant must provide one of the following that carries a signature, and one that carries a photograph that can be matched to the applicant.

  • Drivers license
  • Public Service Employee Number
  • Centrelink card
  • Tertiary Education ID card
  • Proof of Age card
  • Passport (current or expired in last 5 years)
  • Birth certificate (or extract)
  • Citizenship certificate (or certified paper
